Our interdisciplinary team works together with each resident’s doctor, therapist and other healthcare professionals to develop an individually designed specialized care plan to meet the specific needs of each resident. This plan is implemented by our dedicated and professional staff.
The extensive six-day-a-week activity program is organized and directed by our nationally-certified activity director and an assistant activity coordinator. There is always a standing invitation for families and friends to take part in all activities with their loved ones. We offer religious services of different denominations throughout the week. Scheduled outings give our residents a chance to remain part of the community. In addition, a large volunteer program provides other daily events for our residents’ pleasure. All of our activities are designed toward developing and maintaining a high level of independence and quality of life for each resident.

Weston Place at Town & Country is a 10 bed skilled nursing facility for short-term rehabilitation. It was designed as a transition for people who just need a little time and rehabilitation before they are ready to go home.
Residents may be referred to Weston Place at Town & Country by a physician or case manager or can be referred by themselves or family members.
Weston Place at Town & Country accepts Medicare, private pay and most private insurances. Our admission office can confirm your insurance coverage.
